1. Terms and conditions apply to all sample requests. 2. Sample requests are not open to anyone under the age of 16. 3. No cash alternatives. 4. Entering your details on-line is deemed to be acknowledgement of these terms and conditions and agreement bound by them. TePe will only use the data you provide for the purposes of administering the free sample. 5. The sample request can be withdrawn anytime. 6. Free samples are dependent on stock levels. 7. One sample request per individual. 8. Those requesting samples must have received the periodontitis and diabetes leaflet. 9. TePe reserves the right to refuse any sample request.
